On October 1, 1994, the 445th AES was activated concurrently with its parent wing, the 445th Airlift Wing. Prior to that date, the unit’s members and materiel were assigned to the 67th Aeromedical Evacuation Squadron under the 907th Airlift Group and the 906th Fighter Group.
The 445th AES members have participated in a wide variety of both peacetime and wartime missions, including: Operations Desert Shield and Desert Storm, Operation Just Cause in Panama; Operation Restore Hope, the relief effort in Somalia; Operation Uphold Democracy in Haiti; and Operation Joint Endeavor/Guard/Forge in Bosnia.

The 445th AES has volunteered to support and fly a significant portion of world-wide aeromedical missions. The squadron flew over 150 operational and training missions, performing over 650 flying hours during the 2000 year. This included missions throughout the continental United States, the Pacific and European theaters, South America and the Caribbean.
The 445th AES’s primary aircraft is the C-141C Starlifter.

The mission of the 445th AES is to deploy aeromedical evacuation crews highly trained and equipped to provide inflight medical care aboard mission directed aircraft used to airlift patients. It is also tasked with deploying highly trained aeromedical operations personnel to provide operational support and mission management at aerial ports supporting aeromedical evacuation operations; and with deploying highly trained aeromedical crew management personnel to provide operational management of attached, assigned and transiting aeromedical crews at aerial ports supporting aeromedical evacuation operations.

The squadron is ready and capable of supporting aeromedical evacuation on most Air Force aircraft including; the C-130 Hercules, the C-9 Nightingale (the only aircraft in the USAF transport inventory solely dedicated to aeromedical evacuation), the C-5 Galaxy, the C-17 Globemaster III, the KC-135 Stratotanker and KC-10 Extender should conditions warrant their use. In addition, the unit is capable of supporting Civil Reserve Air Fleet aeromedical evacuation using Boeing 767-200 and 767-300 aircraft.
